By checking "I agree to the authorization agreement for direct
payment (ACH Debits)," you are indicating the following:
- You authorize General Casualty to collect the amount(s)
specified from the checking or savings account at the depository
institution that you indicated, for payment of your insurance
premiums.
- You understand that if your payment is returned to General
Casualty due to insufficient funds in your bank account, the
payment will be reversed and a penalty charge will be assessed by
General Casualty.
- You represent and warrant that you are authorized to use the
bank account(s) that you have specified on the General Casualty
QBEpay service.
